Pricing and Insurance

To provide the best service we know how, Golden Apple MD & Nutrition operates under a concierge model.

We care for patients who do, or do not, have insurance.

Giving good care, and advocating for our patients is important to us. Being available for patients for visits and follow-up is also important to us. Good medical care is about good follow up, and being available for patients.

We are also skilled at providing care for patients without insurance. Too often the medical field abandons uninsured patients. We feel ALL patients are deserving of good care. We will discuss out-of-pocket costs for labs and imaging, and are often able to find lower cost options. Our published concierge prices are the same insured and for non-insured patients.

Many insurance providers cover necessary lab tests. We are skilled at ordering labs and getting them covered by your insurance to the extent that this is possible. We discuss any possible lab fees during your visit, so our patients can make informed decisions regarding lab costs.

PRICING INFORMATION for Dr. Debrier’s concierge model

1) $500 first month initial consult fee

-- includes one 60-90 minute intake visit which includes:

● Thorough discussion regarding medical history and family history, including individual risk factors, issues, and goals, and prescriptions for each issue.

● Prescriptions for medications for all issues discussed, including GLP medications. *

● Ordering and discussing lab work, ordering and discussing, imaging if needed (such as MRI, X-ray or mammogram) Not every patient needs bloodwork! We order blood tests when it makes sense. We work with your insurance company to get labs and imaging covered, and we let you know in advance when we are ordering a non-covered test.

● Your initial consult can focus on general health, menopausal hormone issues, any medical issue such as diabetes, hypothyroidism, weight loss, high blood pressure, incontinence, or psychiatric issues.

● Dr. Debrier does see patients for psychiatric issues, including patients currently taking psychiatric medications, or patients wishing to discuss starting new psychiatric medications., and can do in-depth visits regarding psychiatric issues and possible psychiatric medications.

Details regarding prescription medications:

* Note that due to the multistate dimension of her practice, GoldenApple does not prescribe controlled substances-- including ADHD medications, such as Ritalin, Adderall, Concerta or Vyvanse, or controlled anxiolytic medications such as Xanax or Ativan. We also do not prescribe Opiate medications. We do prescribe non-controlled pain medications.

**All other psychiatric medications are included, such as SSRI medications-  Lexapro, Paxil, Celexa or Zoloft, and also does include mood stabilizer medications such as Lamictal or Carbamazepine or Gapapentin 

2) $150 monthly fee ongoing

● Includes a one-hour monthly visit, with new prescriptions if needed, and refills for ongoing medications

● Yes, we offer acute care visits for our patients, for situations such as the dreaded ‘I-am-travelling-out-of-state-and-just-got-a-UTI/Ear infection/Bronchitis’ situations. We of course also provide acute care visits when our patients are in town.
