A small patient panel means we know you and we are available for you
We treat patients like the small family they are. We will get to know you. We want you to feel comfortable calling us for medical and nutritional advice. We think reaching your doctor or nutritionist shouldn’t be difficult or scary.
Transparent pricing:
Our concierge patients pay an initial consult fee of $500, and a monthly fee of $250, which includes all visits, texts, and emails.
Most visits are available in 6-8 hours. Our monthly fee INCLUDES unlimited visits for you, 6 visits per year for your (female) children. Because we want to make your life easier and we know it can be difficult to get convenient care for your kids, especially from a trusted provider who knows your family.
All of our services are available over the phone or online, whether consults, coaching or follow-up appointments. Any necessary testing (including blood draws, mammograms, MRIs, etc.) can be done at a local facility, and we will work with you to find the best facility for your needs, and to have your testing covered by insurance.

It’s essential for us that our clients get accessible and efficient care. We know you are busy. We know some days (most days?) you are disorganized and rushed. We know you go on vacation and forget your meds. We know your health and nutrition get derailed by life. We know you are not perfect. We know you forget to fill your medications until the last minute. We are here to help and “have your back” not judge. We’ve created a system and set of protocols that optimize services for our busy patients,when and how you need them, without all the bureaucracy of a traditional doctor’s office. PRICING INFORMATION for integrative Concierge care with Dr. Lesley Debrier and Danielle Debrier, RD
1) $500 first month initiation fee, then $250 monthly ongoing. Cancel anytime.
Your monthly fee includes 6 visits per year for your (female) kids over age 6.
Here are the details:
In your first month, we invite you to schedule a 60 minute intake visit which includes:
● Thorough discussion regarding medical history and family history, including individual risk factors, issues, and goals, and prescriptions for each issue with Dr. Debrier and with Danielle, RN in separate visits. This visit is not mandatory. We know you are busy.
● Prescriptions for medications for all issues discussed and filled, including GLP medications. *
● Ordering and discussing lab work, ordering and discussing, imaging if needed (such as MRI, X-ray or mammogram) Not every patient needs bloodwork! We order blood tests when it makes sense. We work with your insurance company to get labs and imaging covered, and we let you know in advance when we are ordering a non-covered test.
● Your initial consult can focus on general health, menopausal hormone issues, any medical issue such as diabetes, hypothyroidism, weight loss, high blood pressure, incontinence, or psychiatric issues.
● Dr. Debrier does see patients for psychiatric issues, including patients currently taking psychiatric medications, or patients wishing to discuss starting new psychiatric medications, and can do in-depth visits regarding psychiatric issues and possible psychiatric medications.
Details regarding prescription medications and forms:
* Note that due to the multistate dimension of her practice, GoldenApple does not prescribe controlled substances-- including ADHD medications, such as Ritalin, Adderall, Concerta or Vyvanse, or controlled anxiolytic medications such as Xanax or Ativan. We also do not prescribe Opiate medications. We do prescribe non-controlled pain medications.
*Note that we are not able to fill out disability forms or FMLA paperwork.
**All other psychiatric medications are included, such as SSRI medications- Lexapro, Paxil, Celexa or Zoloft, and also does include mood stabilizer medications such as Lamictal or Carbamazepine or Gapapentin
ALL visits are covered by the plan. All text messages, emails, and phone calls are covered by the plan.
