This means care for patients with insurance, and those without. We strongly support the “No Surprise Billing” policies currently being legislated in many states. As an independent medical practice, we treat patients by providing direct care, medication prescriptions and management, nutrition planning and follow-up, and more. All of our services are available online, whether consults, coaching or follow-up appointments. Any necessary testing (including blood draws, mammograms, MRIs, etc.) is handled by reputable providers.

Insurance and billing

To provide the best service we know how, Golden Apple MD & Nutrition accepts all major private insurance plans, so these patients will only pay their typical copay amount.

We also care for patients who don’t have insurance.

Also, we strongly support the “no surprise billing” policies currently in legislation in many states. This leads to our simple billing promise- for insured patients, you will only pay your copay at the time of visit. There will be no further billing. Yes, you read that correctly. No. other. after-visit. bills.

We are so confident in our billing team that, if your insurance denies payment for your visit-we go back to your insurance, not you. In the unlikely event that we are not able to obtain insurance reimbursement for your visit, for any reason-“the buck stops there”. We will not bill you the patient. Giving good care, and advocating for our patients is important to us. More important than saddling our patients with surprise after-visit bills from our office.

We are also skilled at providing care for patients without insurance. Too often the medical field abandons uninsured patients. We feel ALL patients are deserving of good care. We will discuss out-of-pocket costs for labs and imaging, and are often able to find lower cost options. Our published prices are for non-insured patients.

However, we do offer hardship rates, please contact our office and answer a few simple questions to qualify. We will not ask for personal information such as bank statements, tax returns, or paystubs for patients requesting hardship rates.

Many insurance providers cover necessary lab tests. We are skilled at ordering labs and getting them covered by your insurance to the extent that this is possible. We discuss any possible lab fees during your visit, so our patients can make informed decisions regarding lab costs.

We understand that this form of honest and transparent billing is new to many people, but the need for cost transparency in health care becomes more urgent every year. We’re happy to discuss this process in greater detail during your next appointment.

Appointments and Rates

Our appointment options are provided below depending on the type of appointment and length of the visit. All appointments are online. Although we do not offer in-person services, we try our best to assist you remotely in a way that is as comprehensive as it is convenient. All appointments are by phone but you may request a video appointment.

Primary Care Visit

Nutritional Initial Consultation

This is for existing patients, and new patients, as available.

  • Simple visits - for one issue or people taking up to three medications

    • 30 minutes | $150

  • Complex visits - for multiple issues, or people taking more than three medications

    • 60 minutes | $275

  • Follow up visit

    • 30 minutes | $150

  • We will ask for the medications you take before your appointment and if you have a medication we are unable to refill, we will let you know prior to your appointment.

  • We cannot prescribe or refill narcotic pain medications.

  • We do offer hardship rates. We realize these are challenging time for many. Please contact our office to request. All government workers affected by any shut-downs or other issues will be offered hardship rates, as well as first responders

  • The first visit will be 60 minutes, we will spend time discussing your healthcare history, family health history, current symptoms and issues, and health goals. We will then discuss several potential treatment options-including the benefits AND risks. We believe women should have all of the facts, not just some of them.

MORE INFORMATION-

There is no "one size fits all" for women's hormones, so will design a customized health and medical program that is fit for you, with various options discussed, and of course each patient will make her own decisions about options. We are here to work with you, not dictate to you.

This visit includes prescriptions for hormonal and/or non-hormonal treatment to be sent to your pharmacy electronically during the visit, then you just go pick up your medications at your local pharmacy.

Yes- most hormones and other prescriptions will be covered by your health insurance

Yes we DO discuss costs-some medication options cost more than others, even with insurance, and we will let you know about cost differences.

Yes you CAN do all of this without insurance, we can also discuss out of pocket costs for hormones

Out of pocket costs (meaning costs for those of us without insurance-are usually $30 per month at your pharmacy but costs vary and we WILL tell you costs

NO we don't sell or market ANY hormones, supplements or other products. We have NO AGENDA and NO BIAS. We do NOT receive any outside compensation or 'kick backs' of any sort. We do not do endorsements. We are not "influencers". Our goal is to give women good quality care, and that's it.

YES we can order labs through your insurance, we will discuss what labs will be ordered, and then discuss lab results with you, and you will get a copy of your results over email for your records (labs are usually optional for hormone treatment, and yes we can do saliva hormone testing for those who are needle-phobic!)

YES we can order imaging, such as a mammogram (again imaging is optional)

NO, sorry, we can not prescribe or refill custom compounded hormones, please continue to see your naturopathic or other provider. However, we can have an in-depth conversation about converting your current custom compounded hormones to similar, FDA approved hormones, each patient can then choose if she would like to continue her current custom compounded hormones or switch

YES we can manage and follow all thyroid conditions, including over- or under-active thyroid issues, YES we can proscribe Armour or NP Thyroid (also know as Nature Thyroid). We will help you with your thyroid issues without judgement, we look at both numbers and the whole person including symptoms

Menopause visit costs: $275 for first 60 minute visit, then $150 for 20-30 minute follow up visits, to be done every 3-6 months. We request a minimum of two visits per 12 months.

This hour-long session will be spent evaluating your physical health, statistical chart, meds, diet and supplement history along with discussing your health and nutrition goals in order to build and maintain an integrative, evidenced-based health and wellness program for you. This includes any post-appointment work and communications by our integrative nutritionist.

Follow-Up Nutritional

Either as a stand-alone session or in conjunction with ongoing services, this consultation will continue any one-on-one dietary guidance and advice, and to monitor an established program.

Weight Loss Visit

The first visit will be 60 minutes, and we will spend time discussing your healthcare history and family history, current weight and future weight goals. We will then discuss treatment options, including risks and benefits

MORE INFORMATION:

We do NOT prescribe any custom compounded weight loss medications. We do NOT sell them out of our clinic. All prescriptions will be sent to your regular local pharmacy, using FDA approved and supervised brand name medications.

We DO discuss costs. We work with manufacturers programs to cut costs. currently Zepbound has a direct-to-consumer program for $550 per month total cost, without insurance, for qualifying customers. We also DO work with insurance to get your medication covered if possible. New pricing offers are ongoing, Walmart is now offering lower rates than Lily in most areas. We expect the cost of weight loss medications to decrease over time.

We DO follow you and offer follow up care and labs when needed.

YES we can order labs through your insurance. YES basic labs are often mandatory, a Complete Metabolic Profile blood draw is the most that the majority of patients require, this costs around $30 out of pocket and is covered at 100% through many insurance plans

Weight loss visit initial cost: $275 for the first 60 minute visit, then $150 for 20-30 minute follow up visits. MONTHLY follow up visits are MANDATORY for the first TWO months, then every THREE months after.
